The Company is seeking a Hospice Care Consultant to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.

Overview
This position contributes to the Company mission of 'Honoring Life and Offering Hope' by interacting and assisting with referrals from healthcare professionals in hospitals, home health agencies, nursing homes, long term care facilities and physicians in private practice.
Positions Over Which Supervisory Responsibility is Held : none
Qualifications
• Bachelor's degree, preferred or 5+ years' experience in hospice, home health, healthcare sales or marketing, in lieu of degree
• Written and verbal communication skills
• Demonstrated ability to work closely with a variety of stakeholders
• Problem-solving skills
• Understanding of referral patterns
• Experience with, or willingness to learn, Medicare regulations and related reimbursement practices related to hospice and long-term care
• Proficiency with Microsoft Office Suite products
• Available for after hours and weekend referrals
• Demonstrate familiarity with policies of the agency and state(s) and federal regulations pertaining to hospice services
• Ability to work as part of an interdisciplinary team
