We have an opening for a Clinical Operations Assistant (LPN or CMA) .  This is a clinical office-based role supporting our provider teams in the field who are traveling to assisted living and memory care communities where they provide primary care services to members within the comfort of their own homes.
This role ensures daily clinical and administrative functions are completed to support the team's time-sensitive needs as outlined further below.
This is an office based role located in Tampa, FL.

Responsibilities:
• Processing medication prior authorizations for primary care teams
• Communicate with specialty providers, pharmacy, patients, families, the State, or legal guardians to coordinate patient care needs
• Act as a liaison in the office supporting Provider field staff members
• Ensures electronic medical records are accurately updated on a daily basis, including setting up e-prescriptions under direct supervision of providers
• Data collection and documentation of all clinical-driven Meaningful Use and other quality and regulatory requirements
• Medication reconciliation
• Data entry such as diagnostics and lab input
• Transitional care management documentation and tracking
• Monitor patient portal messages, chart preparations, and summarize for providers as necessary
Qualifications:
• Graduate of an accredited Medical Assistant Program or Licensed Practical Nurse Program
• Active MN LPN License required - or - medical assistant certification or registration through accredited program required
• Minimum 1-2 years having worked in a clinical setting
• Medical terminology knowledge and understanding of patient care notes
• Technology and Computer proficient, electronic medical record systems experience preferred
• Excellent interpersonal and customer service skills
• Detail oriented and accurate
• 45+ WPM typing,
• Demonstrated compatibility with Bluestone’s mission and operating philosophies
• Demonstrated ability to read, write, speak, and understand the English language
