Hawthorne Health and Rehab of Brandon - Job Function:           To provide Physical Therapy Services   Supervises:               No one   Reports To:               Therapy Director    Hours: PRN     Primary Responsibilities:   1.        Coordinate with therapy team to schedule screenings and treatments for residents on caseload. 2.        Treatment of residents per Physician’s order. 3.        Completion of all documentation related to plan of care including; screenings, progress notes, reassessments, D/C summaries, etc. 4.        Cooperate with facility staff to develop programming consistent with facility/resident needs. 5.        Provide facility in-services pertinent to staff education in specialized therapeutic areas-coordinate with Administrator. 6.        Attend facility meetings/in-service requested –per the Therapy Director. 7.        Follow all facility policies and procedures with respect to state/federal guidelines. 8.        Other duties as assigned by the Therapy Director.     Physical Demands:   The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.    The employee must occasionally lift and / or move / push and / or pull up to 50 lbs.
