Driver's License Social Security Card CPR Card
CNA License or Home Health Aide Certificate
Current Auto Insurance
Results of a TB or Chest X-Ray (completed within the last 24 months)
3 CEU's: Alzheimer/Dementia, HIV, and Assisting with Medications
