Full Job Description Job Function: Coordinate all services provided on Garden Court/Memory Lane and comply with all regulatory agencies and administrative direction.
Primary Responsibilities:
1. Recruiting, hiring, orientation, training, and scheduling of staff.
2. Screen prospective residents; serve as member of Admissions Committee for admission of residents to Garden Court/Memory Lane.
3. Act as liaison between the facility and resident/family members.
4. Complete assigned Observations and Progress Notes.
5. Coordinate programs, activities and social events
6. Ensure that the social needs of the residents and family members have been met.
7. Coordinate volunteers and community integration.
8. Assist in the development of the service plan
9. Perform other duties as assigned by the Supervisor.
Specific Duties:
1. Complete CNA/PA schedule.
2. Manages CNA/PA call offs and shift changes.
3. Ensures all shifts have been covered adequately. Communicates illnesses, appointments, etc. to staff.
4. Direct and supervise CNAs/PAs on their respective shift.
5. Ensures that residents have received AOL cares, activities have been implemented and the residents have been encouraged to attend.
6. Supervise dining room service to make sure:
a. Beverages are passed
b. Residents are monitored and assisted as necessary
c. Residents are transferred to dining room chairs
d. Residents are cleaned, directed, or transported out of the dining room.
7. Ensure that staff is taking scheduled breaks as required.
8. Direct, supervise, educate and provide training to CNAs/PAs throughout each shift to ensure that all direct resident care has been completed appropriately.
9. Make rounds regularly. Ensure that any duties not completed during a shift have been communicated to the oncoming shift.
10. When a new admission arrives, ensure that the room has been prepared for the resident, and the staff is familiar with resident status.
11. Implement all approved resident care programs:
a. Ensure that the resident's service plan is followed when care is provided
b. Ensure that CNAs/Pas are offering fluids frequently throughout the shift and that water is accessible to the residents
12. Plan, develop and monitor implementation of activity calendar.
13. Complete quarterly documentation on activities
14. Complete and review observations as required
15. Ensure all infection control procedures are being followed.
16. Ensure CNAs are attending required/mandatory in-services as scheduled .
17. Screen prospective residents for Garden Court utilizing AL-62 Alzheimer's Screening Form.

While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle or feel; reach with hands and arms; talk or hear and smell. The employee is occasionally required to stoop, kneel, crouch, climb, balance, or crawl. The employee is frequently required to sit. Employee must be able to assist in the evacuation of residents in emergency situations.
Emotional stability to deal with stressful situations on a regular basis; Ability to deal with illness, death and dying.
Qualifications:
A minimum of 5 years of experience working with persons with Alzheimer's disease and other dementia, at least two years of which are management experience working with persons with Alzheimer's disease and other dementia.
