Job Title/Position: Health and Wellness Director (HWD) JOB DESCRIPTION SUMMARY
The HWD is responsible for the overall direction and leadership of resident care in the assisted living and memory care community.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
The Health and Wellness Director must exhibit the following competencies:

SPECIFIC DUTIES AND RESPONSIBILITIES
1. Leadership
• Demonstrates support of the corporate mission (by actions and attitude), policies and procedures and assures consistent compliance within the memory care program
• Establishes departmental goals consistent with corporate goals and objectives.
• Organizes and facilitates regular staff meetings with the goal of positive, motivating team building, as well as staff problem solving and support.
• Facilitates appropriate upward and downward communication.
• Communicates identified needs and potential solutions to appropriate manager.
• Communicates positively with residents, families, and community professionals regarding the community and its benefits.
• Implements corporate policies, procedures, and guidelines.
2. Health and Wellness Program
• Participates in pre-admission screenings of prospective new residents.
• Leads high quality innovative health and wellness programs in the community
• Daily oversight of the operations in the health and wellness department.
• Responsible for implementing resident evaluations and service plans
• Responsible for participating in resident/family care plan meetings and updating the service plan as necessary
• Participates in direct care of resident as needed
3. Human Resources Management
• Recruits, orientates, and trains health and wellness care staff.
• Responsible for overseeing and ensuring new hire orientation and annual training for all assisted living staff, directly or delegated.
• Staffing and scheduling team members in the assisted living to reflect a continuum of care.
• Ensures that regular performance evaluations are held, and that sound human resource practices are in place.
• Encourages staff development and education.
• Maintains a culture, which attracts keeps and motivates a diverse staff of top quality people.
QUALIFICATIONS
• Minimum one (1) years’ management experience in senior living or related health programs preferred.
• Demonstrated effective leadership skills in fiscal clinical management, strategic planning, and program and staff development.
• Must be computer proficient in typing and various programs, including background in EMR.
• Ability to work with and supervise others as an effective team builder and team player.
• Skilled at establishing/maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure thorough understanding of LifeWell Senior Living services.
• Ability to effectively evaluate performance and take corrective actions when necessary.
• Ability to work autonomously, take initiative, set priorities, organize work, and make independent decisions.
• Excellent communication skills, including public speaking, both verbal and written.
• Subject to criminal background check and drug screening.
• Holds current unencumbered license in the state of employed as a Licensed Vocational Nurse or Registered Nurse
• Holds current CPR certification
WORKING CONDITIONS
• Able to work full time with some weekend work
• On-Call per manager schedule.
• This position is paid a salary intended to cover all hours worked during the week, knowing those hours may sometimes exceed eight hours in a day or forty hours in a week and may fluctuate from week to week.
