Full Job Description Summary: To be responsible for providing accurate and timely entry of data into program collection systems and providing overall clerical support for the Nurse Family Partnership (NFP) program.
Duties and Scope of Responsibilities
• Inputs NFP data in a timely and accurate manner into all relevant data management systems; complies with or assists in the compilation of statistical information for monthly, quarterly, yearly and additional special reports (for internal tracking and funding report requirements)
• Collaborates with team members, supervisor, and administration to ensure QI/QA efforts
• Compiles, organizes and reviews data collected by RN's for reports and conferences
• Performs continuous quality improvement initiatives by calculating data, creating the run charts, bar graphs and poster presentations for the Learning Collaborative; collaborates with QI/QA efforts of funding agencies
• Performs and organizes monthly chart audits and uploads monthly worksheets
• Maintains, compiles and organizes forms for multiple electronic data systems
• Performs program support tasks such as maintaining program brochures and forms; photocopies; files; orders and maintains program materials/office supplies and educational handouts; makes reminder calls for visits as requested by the supervisor and nurse home visitors
• Performs general clerical functions such as filing, preparing correspondence, reports and other documents
• Retrieves client referrals as delegated by the supervisor
• Attends meetings, site visits, learning sessions and conferences both locally and nationally (as funding allows)
• Maintains confidentiality of patient records and adheres to HIPAA regulations
• Perform other job duties as required
• Carry out all other responsibilities as directed by supervisor(s)
Qualifications/Requirements :
• Possess a team approach commitment while working with a variety of constituents and community organizations to include RN's, case managers, professional health educators, funders, etc
• Prioritize work effectively in a fast-paced, deadline-oriented office environment.
• Excellent interpersonal, writing and computer skills
• Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs
• Ability to define problems, collect data, establish facts and draw valid conclusions.
• Reliable transportation with proof of valid driver's license and auto insurance or the ability and means to travel as needed for meetings and conferences
Education/Training/Experience
• Proficient personal computer experience to include Microsoft Outlook, Word, Excel, PowerPoint and Internet applications
• High school diploma or general education degree (GED) required. One to two years of college or technical school preferred
