Department Overview:
The Case Management Department is an integrated model with registered nurse and social work case managers. The case managers work together to assess patient and family needs across the hospital. They work collaboratively with all disciplines, patients, and families to achieve mutually agreed upon care goals. Case managers facilitate multi-disciplinary rounds to effectively transition patients safely to the next level of care and manage throughput initiatives. Our emergency department is staffed 24/7 with case managers and we have a dedicated observation management team.
Job Description:
Collaborates with Physician and Interdisciplinary team to determine the correct level of care (OBS, Inpatient), plan of care, treatment, estimated LOS, and likely discharge disposition (home, LTAC, SNF, ALF).  Ensures that Physician/Interdisciplinary team discusses estimated LOS, tentative discharge date and assessed needs for discharge with the patient and family.  Collaborates with Interdisciplinary team to continuously identify more effective strategies to resolve barriers, improve processes and systems, and change practice as indicated.  Supports the efforts of HIM and Patient Accounts by ensuring timely, accurate, and complete data entry in multiple information systems/data bases.  Reviews medical record to anticipate clinical stability and to have a thorough understanding of the patient prior to speaking with the physician and/or participating in multidisciplinary rounds/huddles. Participates in interdisciplinary Rounds or Huddles.  Ensures patient is progressing through clinical milestones and adjusting the targeted discharge day as indicated.   Escalates barriers to the plan of care (clinical, social, environmental) through appropriate channels to resolution.  Works in partnership with physicians to consider alternate levels of care if patient is not meeting acute care criteria.  Facilitates attending physician communication with insurance plan medical director and consults Physician Advisor as appropriate.  Facilitates communication among physicians caring for patient to advance plan of care; Assists Resource Center Associate with any concurrent denial management issues. Monitors and documents denials and avoidable days and communicates anticipated discharge date to Resource Center for those patients requiring Medicare Important Message.  Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital.
• Licensure to practice as a Registered Nurse by the State of Florida.
• BSN required
• Three years nursing experience with at least two years in Case Management OR two years in Emergency Medicine
• Must have excellent computer skills
