**Description** **Responsibilities** The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures. **Required Qualifications** + Licensed Registered Nurse (R.N.) in the state of Florida with no disciplinary action + 2 plus years of case management experience + 3 plus years of experience in a clinical setting to include Med Surg, ICU, Rehab, and/or Hospice setting + Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook + Must be willing to work an 8 hour shift, 40 hours a week, Monday through Friday sometime between the hours of 8:00 AM and 8:00 PM Eastern Time + Strong technical computer skills with the ability to navigate between multiple programs and systems to type notes and update patient information + Strong written and verbal communication skills **Work at Home/Remote Requirements** + Must ensure designated work area is free from distractions during work hours and virtual meetings + Must provide a high-speed DSL or cable modem for a workspace (Satellite and Hotspots are prohibited).
