Job Summary:
Facilitates coordination, communication, and collaboration on behalf of the individual(s)/ guardian(s). They assist in accessing community resources and services. They assist individual(s)/ guardian(s) in applying for Kentucky waiver services and Medicaid.
Minimum Qualifications:
+ BS/BA degree in Psychology, Sociology or Social Work or other Human Services degree approved by DDID, or RN licensure.
+ One (1) year experience in working with individuals with developmental disabilities.
+ Valid driver's license and the ability to provide safe and reliable transportation.

Relocation is not provided and travel is required
