Description
The Care Coordinator is responsible for facilitating the care of injured workers with the provider for Paradigm Specialty Network (PSN) referrals. Acts as a liaison with the injured worker, client, and provider to assure the best and speediest care is provided. Completes all reporting requirements to fulfill client’s expectations while delivering excellent customer service. Handles phone & email inquiries from providers and/or clients with questions regarding claimants care and work status. Have an outstanding attention to detail, question things that do not appear correct and possess a desire to succeed and progress quickly.

Requirements
Handle PSN referrals by providing a consistent process for the coordination of care
• Provide exceptional customer service
• Obtain medical notes and provide timely reporting to client
• Schedule and confirm injured workers appointments and provide client with status updates
• Provide exceptional customer service while providing phone support and email communications with both clients & providers.
• Work closely with interdepartmental operations staff to effectively manage referrals.
• Complies with and adheres to all regulatory compliance areas, policies and procedures and best practices
• Understand & comply with all HIPAA & SOC2 requirements necessary in working with designated PHI.
• Perform additional support functions as needed.
Qualifications
Excellent customer service skills.
• Excellent oral and email communication skills and phone presence.
• Ability to work independently.
• Acute attention to detail.
• Proficient in all Microsoft Office and computer application systems.
