EXERCISE REFERRAL SPECIALIST Clinical Support Hours: 8a-4:30p/Variable Days - Full-time Paygrade/Wages/Salary Info: OH 2013-73 - 40075B
Job Summary Responsible for distribution of Exercise is Medicine (EIM) information and referral tools to local healthcare providers and coordinates all referrals to ensure a positive experience for patients and increase membership. # Job Responsibilities Assists with organization of specially assigned programs in the fitness areas for the member population. Responsible for the day-to-day and long-term implementation of EIM programming. Responsible for presenting special programs, events, and classes at the HealthPark, in the community, schools and for business # industry as needed. Acts as point person for medical emergencies, opening/closing, minor problems, member concerns and contact of Manager on Duty when situation warrants. Assists in training all Exercise Physiologists who will be working with the EIM program and/or other specialty programs. Ensures electronic documentation is completed for referred patients and assists with training and questions related to EPIC. Monitors program participation and ensures electronic documentation is completed for referred patients. Assists with troubleshooting simple electronic documentation errors and questions and routes more complex issues to appropriate IT support. Develops a readiness to change interview tool to be administered to referred patients and interviews all referrals to determine if they are qualified to participate in the program(s). Performs fitness testing and exercise prescription based on the needs and goals of each client. In working with physician-referred clients, the exercise physiologist will not only assess and prescribe appropriate exercise, but will coach them and track their progress, following up with the member and the physician. May be responsible for documenting and entering charges in an electronic medical record system. Recognizes contraindications to exercise and abnormal patient responses to exercise. Responds appropriately to all situations including exercise cessation and referral to medical professionals. Collects pre and post data from outcomes and compiles reports for HealthPark Leadership and other parties as appropriate. Creates reports to track participation rates and rollover. Responsible for greeting every member and potential member with a smile and making themselves available to help meet their needs, answer their questions and make them feel important. Performs fitness assessments and other evaluations specific to participants. Works with leadership team to assist and coordinate special events related to programming. Provides exercise information for patients, peers and the community which may include public speaking and education classes. Must be able to work in a stressful environment and take appropriate action. Experience with direct accountability for programs, fitness testing, exercise prescriptions and customer service preferred.
