Aledade is currently looking for full-time temporary support for a Patient Outreach Specialist. In this role, you will be conducting emergency department outreach calls, provide patient education about clinic appointment availability, and identify and communicate if the patient has any care coordination needs or triaging needs from the supported practice. All work will occur in partnership with the market and practice leadership team. As a Patient Outreach Specialist, you will be expected to work 25-40 hours per week for roughly 3 months. This is a remote/telecommuting position must work east coast business hours.
The duties of this position are: • Conduct direct telephonic outreach to patients on the provided list.
• Follow standard messaging provided by Aledade, and assist patients in setting up an appointment with their PCP.
• Apply motivational interviewing tactics to break down psychosocial barriers to appointment creation and attendance.
• Track outreach and outcome documentation in the Aledade App and/or in a shared Google Drive document or other tracking system provided by Aledade and on Aledade systems.
• Identify outreach trends and cycle back learnings to the team and share creative ideas for increasing patient visits. Retain some degree of flexibility as outreach strategy may shift with greater data availability.
Experience: • 1-year health care experience required
• Minimum 1-year customer service and telephonic experience preferred
• Medical Terminology certification is preferred
• Strong organizational skills
• Ability to work very independently Proficient with Microsoft and Google applications
• Strong communication skills
• Strong motivational skills including conflict/dispute resolution techniques
• Must have access to strong internet connection
