LPN III responsibilities providing skilled home visits:
• Works in collaboration with and is responsible for the delivery of patient care to assigned patients under the supervision of a Registered Nurse.
LPN III responsibilities in the Intake role:
• Documents and processes all referral information, codes, disseminates, and records activities related to referrals.
