In partnership with the Isle of Wight NHS Trust we are seeking 3 people to join our Onwards Care and Independence Team who provide short term (two week) care and support to people in their own homes who are being discharged from hospital, or diverting people from being admitted to hospital.
We are seeking 3 support workers who will work closely with the Advanced Care Practitioner, Patient Pathway colleagues and the Regaining Independence Service colleagues to support people to recover and stay well. You will play a key role in delivering a high quality, personalised services, that supports people with a range of daily living tasks, so they remain at home.
Although experience is not necessary, candidates will be expected to be flexible and will demonstrate a good understanding of working in a community setting as well as working in partnership with others.
As part of our team, you will benefit from full training, full support with the opportunity to advance your career, paid travel time and a generous leave entitlement. You will have the use of pool car for each shift as well as guaranteed hours per week plus overtime.
You will be working on a shift basis across the island with your start location being either Newport or Ryde.
