Position Summary
As a Case Manager with Family Treatment Services, you will oversee patient case management by completing the following:
• Conduct patient intake, screening, orientation, assessment, and evaluation
• Develop and implement initial, transition, and discharge patient planning to address stable housing, gainful employment, engagement in behavioral health support, family needs and resources, and linkage to ancillary services
• Facilitate life skills groups, breath alcohol testing, and urine drug screenings as needed
• Prepare and maintain appropriate clinical records and documentation within agency and state-prescribed timeframes
• Transport patients to community-based appointments in agency vehicles as needed
This is a full-time opportunity, Monday-Friday, with flexible hours available between 8am- 6pm. Minimal weekend/on-call availability may be required.
Qualifications
• Bachelor's degree in a Human Services field
• Experience in the Behavioral Health field and/or Case Management
• Ability to effectively communicate and interface with multiple systems of care
• Computer efficiency, to include Microsoft Suite and familiarity with Electronic Health Records (EHRs)
• Florida CBHCM certification preferred
• Bilingual-Spanish preferred
