Full Job Description We are seeking to hire a highly trained certified medical assistant ( must be Certified) to work as a Chronic Care Manager. The Care Management Program Specialist is responsible for ensuring the successful onboarding of new providers and patients as they implement Chronic Care Management and Transitional Care Management Programs. On-boarding is the most important phase of the client life cycle, and the Care Management Program Specialist is responsible for building and maintaining the client's satisfaction.
The job responsibility consists of the overall administration of the care management program and specific services for patients enrolled in the CCM program. The CCM Care manager manages identified patients through routine "NON-FACE-TO-FACE" telephonic and electronic communication based on needs identified in the individualized comprehensive patient Care Plan. The CCM Navigator will demonstrate performance consistent with professional standards of practice.
** Training will take place over Zoom for 2 weeks. You will then be relocated onsite after training is completed and Covid restrictions are lifted.
Position Requirements:
• Certified Medical Assistant.
• Effective oral written, and excellent phone communication skills.
• Must be able to drive.
• Works well independently with strong organization, collaboration, and time management skills.
• Knowledge of clinical practices, diagnostic, treatment procedures, and billing background to be able to check for medical insurance reimbursement for chronic care management for Medicare patients and copays under any secondary insurance.
