*This position will serve clients in Daviess, McLean, Hancock, and Ohio counties.*
Job Summary:
+ The Hi-Fidelity Wraparound (HFW) Case Manager assures that care is delivered in a manner that respects family culture, supports voice and choice, amd maintains a strength-based focus throughout the HFW process.
+ Offers consultation and education to all providers regarding the values of the wraparound process, monitors progress toward goals, and assures all necessary data for evaluation are gathered and entered.
+ Responsible for completing a comprehensive strengths-based assessment of individual and family, working in full partnership with team members to develop and implement a service plan, identifying providers of services or natural supports, facilitating team meetings, making regular home visits, monitoring all services, and maintaining fiscal accountability.
+ Helps the youth and family develop and integrate their natural support system and manage their own services, supports, and plan.
Minimum Qualifications:
+ Bachelor's degree in a human services related field.
+ One (1) year of experience providing TCM services to children/youth with SED and their families.
+ Willingness and ability to complete HFW Case Manager Training.
+ An understanding of, and experience with, different child-serving systems, including education, behavioral health, child welfare, juvenile justice, health, and others.
+ Experience working with, and genuine respect for, families of children/youth who have complex behavioral needs and a firm commitment to empowering families.
+ Extensive knowledge of community resources
+ Must be available to work regular work hours, as well as some evenings and weekends.
+ Must possess and maintain valid driver's license and have access to reliable means of transportation for job-related use.
