Hiring Salary/Salary Range: $27.00/Hr.
DUTIES:
• Plans, organized, and implements counseling services for patients and families in the USF Diabetes and Endocrinology Center. Working with our clinical social worker, presents professional and patient awareness education and training, including developing, and providing materials.
• Interviews patients and family members to obtain information on home environment, family relationships, employment history, health history, insurance coverage, and patients' personality traits. Assists patients and families to understand, accept, and follow medical recommendations and helps to identify strengths and challenges and works to reduce barriers to care while increasing access.
• Evaluates information obtained, in terms of the medical plan of treatment, and plans a program/action plan that provides maximum benefit. Assists in the facilitation of transition of clinical care from pediatric to adult care providers.
• Working with our clinical social worker, provides information and refers patients and families to community agencies that may assist them. Works collaboratively with diabetes and endocrine care providers, families, and school nursing staff and counselors to provide optimal care to children with diabetes at school.
• Participates in team review of patients’ progress. Documents appropriate information in the patient's medical record.
QUALIFICATIONS (Education & Experience):
Minimum Qualifications:
• Each position assigned to this classification must require specific training at the Bachelor’s degree or Master’s degree level in Education, Special Education, Psychology, Social Work, or other field appropriate to the program content.
• Bachelor’s degree candidates must have at least two years of applicable experience.
• The Master’s degree may substitute for the required experience.
• Specific certifications or licensures may be required or preferred for certain program areas.
Special Working Conditions:
• Evening and weekend work may be required at times, as well as on-call duty for emergencies.
