Role: Under physician's orders and RN supervision, the Hospice House LPN is responsible for the observation of patient/family needs and providing palliative and supportive care to the patient/family unit. As outlined in the plan of care, s/he utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. S/he will report, to the appropriate RN, any change in condition of the patient or family unit. S/he provides emotional support during times of crisis to the patient and family unit.
Qualifications:
• Current LPN licensure in the state of Florida, or current licensure from another state plus a temporary Florida permit.
• Valid driver's license and automobile insurance in the State of Florida.
• Reliable transportation to meet visit schedule.
• Minimum one (1) year nursing experience in an in-patient hospice, or home health setting.
• Certificate of completion of 30 hour Infusion Therapy Course preferred.
• Ability to communicate in English.
• Flexible schedule to meet the needs of hospice patients.
• Ability to use equipment with visual and auditory mechanisms.
• Ability to physically access any home in any service area, i.e., navigate stairs, and narrow spaces, tolerate heat, lack of air conditioning, pets and smoking in any area Company serves.
• Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
• Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
• Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves.
• Ability to physically access patient rooms and other areas of the Hospice House
Job Responsibilities: Clinical
• Under the supervision of an RN, provide palliative nursing care to patients and families incorporating psychosocial, cultural, physical, spiritual and biological components and appropriate nursing intervention as identified in the plan of care developed by the IDG.
• Demonstrate clinical skills regarding application of HPC Clinical and Administrative policy and procedures, such as: medication administration, symptom management, and patient/family relationships.
• Observe and report signs of adverse reactions; response to treatments and results of dressing changes.
• Consult and collaborate with other professionals to promote quality and continuity of patient/family care.
• Document observations/interventions to support higher level of care.
• Attend patient deaths; prepare body for transport; oversee disposal of medications.
• Provide bereavement support to the patient caregiver/family.
• Accept responsibility and is accountable for nursing actions; demonstrates an understanding of legal consequences of nursing actions; requests supervision when needed; performs within the guidelines established by the Florida Nurse Practice Act.
Administrative
• Document observations and interventions on appropriate Company forms with accuracy, timeliness, legibility, professional terminology and awareness of legal and regulatory implications.
• Incorporate behaviors supportive of Internal Mission Statement.
• Perform within the framework of the philosophies, policies, and procedures.
Communication
• Communicate accurately and completely to staff members, patients, families and supervisors; utilize positive approaches when working with other staff members.
• Present concise, pertinent oral and written patient information to team; respect input from all disciplines.
• Listen to voice mail report prior to visiting the patient.
• Report changes in patient condition to RN.
Professional Development
• Pursue own professional growth through attendance at required in-services and other educational offerings.
• Establish professional goals for self and continually evaluate self in the attainment of these goals.
Financial
• Utilize supplies, equipment, medications, laboratory and patient therapies within the HPC philosophy.
Safety
• Practice safe and proper body mechanics relative to moving, positioning and transferring patients.
• Use necessary precautions with communicable diseases.
Other
• Duties as assigned by the Continuous Care Manager.*
