Overview
The Care Promoter Medical Assistant (MA) is a national certified medical assistant who performs routine administrative and clinical responsibilities to support Sanitas’ patient care education programs in the medical centers under the direct supervision of the care educator registered nurse (RN) in accordance with state's practice laws.
Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Coordinate enrollment of patients referred to the program by the PCP through the EMR Dashboard
• Perform patient follow up via email, Face to Face visit, and/or telephone call
• Schedule follow-up visits as recommended by policy
• Document and update patient information into the care program’s data base
• Assist Care Educator Nurse with any task assigned to support the program including but not limited to: • Care Gaps procedures identification, promotion and scheduling
• Perform Retina evaluation
• Collection of A1c / Urine microalbumin
• Source through a list of abnormal A1C values each month to reach out to this population and offer them in the care program.
• Schedules patients for educational group workshops and classes.
• Assist in health fairs and events
Qualifications
KNOWLEDGE, SKILLS AND ABILITIES:
• This position requires some local travel to assist in covering other facility within the same area.
• Must always have excellent oral and written communication skills with evidence based data and etiquette
• Ability to think on their feet to solve problems for patients while achieving care program team goals.
• Must be detail-oriented to ensure accuracy of reports and data.
• Ability to maintain effective and organized schedules to ensure timely patient flow.
• Ability to manage stressful and fast passed work environment
• Bilingual in Spanish and English Required
EDUCATION AND EXPERIENCE CRITERIA:
• High school diploma or equivalent and graduation from a nationally accredited Medical Assistant program with active MA national certification is required.
• Two (2) years Medical Assistant experience in primary care setting is preferred.
• Experience working with chronic diseases education management is a plus.
• CPR/BLS for Healthcare Providers is required.
