PATIENT EDUCATOR - DIABETES - OWENSOBORO HEALTH REGIONAL HOSPITAL Nursing Hours: 7am - 3:30pm Days - Full-time Paygrade/Wages/Salary Info: OH 2013-76 - 40181B
Job Summary Provides diabetes education to patients, caregivers, team members, and the community. # Job Responsibilities Provides diabetes self-management education and training to diabetic patients and their family members utilizing curriculum approved by the American Diabetes Association. Works collaboratively with multi-disciplinary diabetes care team to implement plan of care. Reinforces diabetes education and instruction to staff nurses and support staff. Develops and distributes educational materials to community. Qualifications Bachelor#s degree or Master#s degree in Nursing required upon hire OR Bachelor#s degree or higher in Dietetics required upon hire OR Bachelor#s degree or higher in Nutrition required upon hire A combination of education, training and experience may be considered in lieu of degree. A minimum of 3 years# relevant experience required RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire OR Dietitian - Licensed Dietitian in the state of Kentucky required upon hire AND BLS - Certified as a Basic Life Support Healthcare Provider required within 45 days of employment in this position AND Diabetes - Certified as a Diabetes Educator by the National Certification Board for Diabetes Educators (NCBDE) required within 24 months of employment in this position AND Diabetes - Certified in Advanced Diabetes Management (BC-ADM) by the American Association of Diabetes Educators (AADE) required within 24 months of employment in this position Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
