Qualifications:
• Master's degree or above in Social Work (CSWE accredited); SWs hired before 2009 who have degrees in other fields are grandfathered in; SWs hired before 4/21/2019 who have BSWs are grandfathered in
• Minimum of one (1) year experience in health care/hospital setting, hospice internship, palliative care, or home healthcare
• Familiarity with and working knowledge of the concepts and practices of both adult and pediatric patients: Hospice - death and dying; Palliative - serious illness
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
• Experience in children, family, and group modalities
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
• Able to remain calm, professional, trouble shoot and intervene with patient/family crisis situations
• Ability to effectively communicate in English (verbal and written)
• Ability to work in a team environment and intervene appropriately
• Mobile Driver - Valid driver's license and automobile insurance per Company policy
• Ability to travel to off-site locations
• Ability to physically access any home, hospital, clinic, nursing home, and assisted living facility (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.)
• Has the disposition and personality to work harmoniously with a wide variety of people
• Knowledge of community resources and able to provide assistance to patients and families
• Working knowledge of the Hospice Medicare B

Job Responsibilities:
• Provides explanation of services and assists with advanced care planning. Presents services in an empathetic and compassionate manner.
• Completes admission consents when directed. Handles admissions with excellent customer service and thoroughness.
• Provides emotional support and interventions to patients and families during a crisis at admission.
• Coordinates grief support and bereavement referral for hospice hospital patients and referrals who died prior to admission.
• Provides assistance and coordination with home teams for hospice patients that are transferred back to the hospital and participates with discharge planning.
• Communicates frequently with hospital point of contact RN and/or access team manager to ensure timely follow-up. Discusses any potential needs with point of contact RN/Admission RN.
• Provides complete report to clinical team. Ensures documentation is timely, thorough and complete.
• Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public. Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
• Provides all necessary and timely communication with appropriate clinical staff regarding discharge planning, family dynamics as well as significant social and emotional factors related to health problems and develops a plan to address issues in a nonjudgmental manner.
• Meets established productivity and caseload expectations.
