We are currently looking for a Home Health Care Scheduling Care Coordinator in our Tampa, FL office that shares our dedication to quality and our genuine concern for the comfort and dignity of our clients.
Home Health Care Scheduling Care Coordinator responsibilities include:
• Coordinates and schedules referrals for new and existing home care services.
• Provides continuity of care in coordinating referrals.
• Communicates to each caregiver all pertinent information regarding client care.
• Keeps caregiver calendars and client information accurate and current.
• Communicates to the departmental Director all pertinent information regarding client/caregiver issues.
• Routinely interact with personnel such as clients, family members, and care managers to facilitate home care services.
Comprehensive Ability to:
• Exhibit an eagerness to work in a team-based environment, while taking independent responsibility for all referrals received.
• Accept changes in duties or new assignments with a positive attitude in a fast-paced environment.
• Work with a sense of urgency while maintaining a high level of detail.
• Use language that consistently reflects a thorough understanding of the home care industry.
• Think creatively and independently in referring cases.
Qualifications:
• High School Diploma or GED Required. Some college coursework in healthcare preferred.
• Previous experience in Home Health Care scheduling preferred.
• Strong computer skills with proficient knowledge of Microsoft Word, Outlook and Excel required.
• Must possess strong interpersonal and leadership skills.
• Knowledge of ContinuLink preferred.
• Must be able to solve complex problems and function effectively in a stressful work environment.
• Ability to read, write and communicate in English.
• Bilingual (Spanish/English) preferred

We span across 12 offices covering 48 counties, while actively engaging with over 1,000 qualified caregivers to deliver consistent and personalized care for all ranges of personal and medical needs.
We provide nurses, home health aides, certified nursing assistants and companions who are highly skilled, properly credentialed, and will care for loved ones in the same kind yet efficient manner that they would care for their own families. Sonas fulfills the personal in-home non-medical needs of the aging population and services more complex, skilled-care cases, through Sonas Home Health Care's agencies which are licensed throughout the state.
