Summary
Coordinate case management services to include supportive services with grant partners and community providers, including: workshops, seminars, focus group and activities for the families of at-risk elementary age children, focusing on parent engagement. Assess the social, economic, and supportive services needs and make referrals for appropriate case management services.
Plan, implement, monitor and coordinate all case management activities for education, employment and supportive services. This will consist of developing and maintaining case plans and files, providing feedback relating to program activities, and updating case management activities into the Tracking at a Glance (TAAG) database.
Conduct evaluation of residents using the Family Resource Support Guide tool. Provide needs assessment and case conceptualization that provide an opportunity for residents to reach points of completion towards educational and family success goals. Provide advocacy and guidance to participants of the Village Link Up program to assist with addressing residents' self-development, educational, employability and support service needs for increasing parent engagement and child developmental success.
Essential Job Functions
A. Learn and adhere to Tampa Housing Authority's Policies and Procedures as outlined. Be knowledgeable of changes as instructed by supervisor, local, state or federal partner(s).
B. Case Management Services - Develop and coordinate all case management activities associated with the Village Link Up program and other assigned duties. These will entails conducting assessments, development of a case plan according to the family needs, monitor and evaluate program services and integrate into the case plan(s). The program services and case management process should be factored into the below list of categories:
1. Identify the case management processes that are necessary to determine the assessment of needs, the development of case plans, and plan of actions to assure the integration into case plans.
2. Responsible for conducting the appropriate assessment of families of at-risk elementary school age youth.
3. Attend staff meetings to receive instructions regarding program services and quality assurance of related areas; of education, school attendance, and self-improvement workshops in order to identify needs and barriers to self-sufficiency.
4. Maintenance of case files and recording of client data into the TAAG database involving client eligibility, demographic data, program activities, and use of community services and resources.
5. Complete all appropriate documents, and record casework activities completed on a weekly basis as required to be updated into TAAG.
6. Maintain a continuum of educational and supportive service-related resources, in conjunction with community providers and the Village Link Up program requirements, as needed.
C. Supportive Services - Maintain all functions associated with providing supportive services to the target population and make appropriate referrals to grant partners and other community resources. This will include coordinating the distribution of informational materials pertaining to program services.
7. Workshops and Seminars - Assist with the development and coordination of workshops/seminars and health/social service fairs involving the targeted population to forge linkages to schools and community resources.
8. Maintain case management requirements in accordance with the Village Link Up program scope through case management services, school visits, home visits, and client follow-ups
9. Network and coordinate activities with the targeted schools and a wide variety of area providers and community resources.
10. Participate in the special events planning and implementation of service fairs and special projects.
D. Organize and direct a comprehensive case management plan to provide supportive services for the families of elementary age youth.
11. Organize and direct an on-going program of activities, services and opportunities for programs involving adults and youth.
12. Communicate with supervisor, co-workers, residents, community providers, and general public in a courteous and professional manner.
13. Coordinate case management services with associated program functions, in conjunction with the Village Link Up Scope of Services with efficient and effective work methods and procedures.
14. Define problems, collect data, establish facts, and draw valid conclusions as required.
15. Attend any required training session(s) to increase in knowledge and to improve skills and abilities.
16. Complete any special case assignments as instructed by supervisor and in coordination with community resources.
17. Maintain guidelines of program compliance and other department deadlines.
18. Download daily/weekly case management activities into the TAAG database for timely submissions regarding THA Board Reports and other reporting requirements.
Minimum Qualifications
• Requires Bachelor's degree in social work or related field.
• Requires 2 years of work experience in human services, case management, education, or social work, or an equivalent combination of education and experience.
• Bilingual Spanish preferred.
• Experience in public housing preferred.
• Valid Florida Driver’s License
• Insurable under the agency’s insurance policy
Salary: $41,102
