Children's Targeted Case Manager - Daviess & Henderson Counties

Job Summary:
+ Analyzes family and client situations, compiles and interprets data to complete a comprehensive assessment of psych-social needs for the client/family.
+ Performs face-to-face contacts and needed consultations with other service providers/professionals who are involved with the overall treatment plan of the client. Contacts and consultations are performed during work hours set by the supervisor based on the needs of the client and family
+ Through analysis of the data, client/family contacts, and consultations, the Case Manager will determine if all identified treatment needs are being addressed, identify unmet needs, and provide intervention on behalf of the client/family in the living, school, community, or clinical setting.
+ Facilitates team meetings in which treatment plans/service plans are developed, modified, and reviewed.
+ Will advocate for the client/families interest concerning services, funding sources, treatment/program alternatives, community resources, activities and life choices. Promotes the protection of client/family rights and ensures due process in the event of violations/restrictions of rights. Educates, promotes and encourages clients in the area of self-advocacy whenever possible.
+ Identifies and seeks appropriate resources for resolution during a crisis situation.
+ Collects receipts, manages petty cash, and completes monthly billing forms.
+ Provides documentation of services provided, including assessment, service plan and contact notes.
Minimum Qualifications:
+ Bachelors degree in a behavioral science from an accredited institution. Behavioral sciences include Psychology, Social Work, Sociology, Human Services, Special Education, Family Studies, Counseling, and Nursing.
+ One (1) year relevant experience in children's services or case management after Bachelor's degree in a behavioral science is completed. A Master's degree in a behavioral science can substitute for the one year of experience.
+ Valid Driver's license and ability to provide safe and reliable transportation.
