Position Description
Position Title : Intake Coordinator Location : Tampa
FLSA Designation : Non-Exempt Reports to : Site Manager
Position Purpose: T he Intake Coordinator is responsible for performing all patient intakes while explaining clinic programs, initiating required forms, and answering patient questions.
_EXAMPLE OF DUTIES: _
_ _ESSENTIAL FUNCTIONS

30 % Conducts patient intake while explaining clinic programs and answering all patient inquiries.
25 % Conducts research, validates data, and responds to inquiries from multiple sources including but not limited to patients, nurses, medical providers, operations, and case management.
20% Initiates DH 1628 form and verifies accuracy and completeness.
15 % Performs data entry for all referral, prior authorizations and hospital notifications; attaches and/or includes clinical documents as needed.
10 % Escalates issues to Supervisor, Intake UM as needed.
SUPPORTIVE FUNCTIONS
In addition to performance of the essential functions, this position may be required to perform a combination of the following supportive functions, with the percentage of time performing each function to be solely determined by the supervisor based upon the particular requirements of the company.
• File and store appropriate reports daily.
• Maintain an open line of communication between team members and management.
• Assist office in any other administrative duties as needed.

OTHER DUTIES
Assimilate into the Positive Health Alliance culture through understanding, supporting and participating in all elements of pride and care. Demonstrate working knowledge of the service standards.
Regular attendance in conformance with the standards, which may be established by Positive Health Alliance from time to time, is essential to the successful performance of this position.
SAFETY REQUIREMENTS
Personal Protective Equipment (PPE) may be required when performing work duties that may have the potential of risk to your health or safety. The employer will provide the required PPE. Team members will be trained in the proper use and care of any assigned PPE. It is your responsibility to report defective, damaged, or lost PPE, or equipment that does not fit properly, to your Manager.

• Ability to effectively manage patient and team member concerns in a friendly and positive manner and listening to the nature of the concern, demonstrating empathy with the patient and providing positive and proactive solutions.
• Office machines: Operate calculator/10-key by touch, copier, fax, telephone.
• Ability to multi-task and detail oriented.
• Ability to access and accurately input information into a computer using various computer software programs (Excel, Word, and Bluebook).
• Maintain an open line of communication between patients, team members and management.
• General knowledge of medical necessities
• Ability to listen effectively and to speak and write English clearly.
• Ability to learn and perform all essential job functions accurately and safely with minimal direct supervision, within the initial training period after team member begins work.
QUALIFICATION
STANDARDS
EDUCATION
High School graduate or equivalent preferred.
EXPERIENCE
At least one year experience in related position required.

NOTICE:
Standing, bending, stooping, and lifting weights up to and including 25 lbs. may be required. In addition, a hospitable service atmosphere must be projected at all times.
