Qualifications:
• Master's degree in nursing
• APRN certification recognized by CMS for Medicare billing
• Licensure by state of Florida to work as APRN
• Current DEA license
• Current CPR certificate from the American Heart Association
• Certified by appropriate hospice and palliative nursing organization within two (2) years of hire/job date
• Knowledge and experience in palliative medical practice and symptom management
• Advanced clinical skills which allow thorough patient assessments and management for any patient with advanced chronic disease as well as for those near the end of life
• Ability to work under protocol with multiple physicians in multiple settings
• Basic computer skills including use of email and word processing (Microsoft Outlook and Word or equivalent programs)
• Strong interpersonal skills, communication, collaboration, and conflict resolution
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
• Ability to work independently exercising discretion and independent judgment
• Ability to work in collaboration with other disciplines within the Organization
• Mobile Driver - Valid driver's license and automobile insurance per Company policy
• Reliable transportation to meet visit schedule
• Ability to travel to off-site locations
• Must be able to physically access any home in any area of Company service area (i.e., navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning)
• Ability to exhibit flexibility in schedule regarding appointment changes, care settings, and coverage issues
• Ability to work collaboratively with palliative care interdisciplinary staff, as available, in each care setting
• Palliative Care APRN Preceptor : One (1) year's experience as a Palliative Care APRN preferred, with demonstrated competence in quality clinical management and consistently high achievement of goals, and in depth understanding of Company systems. Must remain in good standing from a performance perspective in providing nursing services to patients. Certification from HPNA obtained within two (2) years of taking the position.

Job Responsibilities:
• Provides high quality direct primary or consultative patient care visits in accordance with productivity standards in all clinical settings as designated by the Medical Director.
• Works directly under protocol with Physicians from the Company's medical staff and reports clinical care provided to the Physician.
• Provides clinical expertise and participates in interdisciplinary team (IDG) decisions regarding diagnosis and medical management of patient/family needs, assuring that the highest quality of medical care is provided and that continuity and comprehensiveness of care is maintained in a cost-effective manner.
• Consults to patients' primary and attending Physicians regarding integration of patients' hospice and/or palliative care and primary or other care.
• Works cooperatively with the Patient Access Department and other staff to facilitate timely evaluation of eligibility for hospice services and provision of services to potential Company patients.
• Participates in after-hour and weekend coverage for patients cared for by the Palliative Care medical staff.
• Reports all administrative activities and completes thorough, accurate and timely medical documentation to support proper CPT and ICD-10 coding on all patients seen at the time of service.
• Actively participates in Company's population health programs and products by assessing and addressing patient's multifactorial needs; developing, overseeing and implementing appropriate plan of care; and providing education and support based on patient's condition, preferences and values.
• Prescribes controlled and non-controlled medications, as indicated, to relieve pain and non-pain symptoms associated with the terminal illness.
• Provides leadership in the development, implementation, and evaluation of standards of practice within the palliative care specialty.
Additional APRN Preceptor Responsibilities:
• Develops and utilizes an orientation checklist for APRNs' first year of employment with the Company and present to the Medical Director for approval.
• Serves as a mentor and provides information and education to the APRNs based on the orientation checklist.
• Develops and updates APRN competency checklist that will be used annually and, as needed, in evaluating the APRNs' performance.
• Assists the orienteer to identify learning needs and provides prompt and consistent feedback, identifying areas in need of further development to the new APRN and the Medical Director.
• Plans, coordinates, and carries out or suggests educational opportunities for the new APRNs in conjunction with the Medical Director.
• Regularly assesses clinical skills of new APRNs over the first year of their employment with the Company.
• Regularly audits new APRNs' documentation of patient visits for accuracy and completeness.
• Maintains records documenting each meeting with APRNs and specific assessments of their progress, recommendations for improvement, and areas still in need of further improvement.
• Provides written report monthly to Medical Director regarding the progress of new APRNs.
• Identifies program issues to the Medical Director that may be helping or hindering new APRNs' learning and on-boarding success.
