Almost Family is hiring a Waiver Case Manager for the Owensboro area.
The Waiver Case Manager – Conflict Free, provides a wide range of services to patients in the home including overall coordination of the person-centered service plan and team.

Additional Details:
QUALIFICATIONS:
1. A Bachelor’s Degree in a health or human services field from an accredited college or university; AND
2. At least one (1) year of experience in a health or human services field; OR
3. Be an actively licensed Registered Nurse (RN) who has:
a. At least two (2) years of experience as a professional nurse in the field of aging or disabilities; OR
4. A Master’s degree in health or human services field from an accredited college or university.
5. Previous experience in home care preferred.
6. Familiarity with Medicaid programs and community resources preferred.
7. Excellent organizational and problem solving skills.
8. Ability to read, write and speak English proficiently.
9. Ability to travel throughout assigned territory, in an efficient manner, on a regular basis.
10. Must be proficient with use of personal computer, including Microsoft Office Suite.
11. Must be flexible with work schedule, including working after normal business hours, i.e. taking calls.
Qualifications:
1.Follows the established patient centered service plan of care and notifies the Waiver Manager of any changes to the patient’s condition, identified needs, or needs for changes in the service authorization or schedule and notifies state and assessment nurse as appropriate.
2. Makes referrals to other health and social agencies, community support groups, and networks to maximize use of all resources.
3. Case Manager will meet with the participant, participant’s guardian, or the participant’s legal representative within seven (7) days of receiving a referral from the independent state assessor regarding the participant.
4. Takes charge of coordinating services through team meetings with representatives of all agencies involved as well as the participant and family in implementing a participant’s person-centered service plan.
5. Establishes a Person Centered Service Plan, processes and enters the Plan into MWMA and updates as appropriate and necessary, minimum of annually.
6. Provides necessary information and support to empower the participant and/or their representatives to direct and control the care planning process and delivery of care.
7. Case Manager will follow up on any critical incident reports to ensure the participant health, safety, and welfare are not jeopardized and document such in MWMA.
8. If an HCB waiver provider involuntarily terminates providing HCB waiver services to a participant, the case manager will provide the participant, participant’s guardian or participant’s legal representative, the name, address and telephone number of each HCB waiver service provider in Kentucky as well as provide assistance in contacting another HCB provider.
9. Contacts the participant at least monthly by telephone or through a face-to-face visit with a minimum of one (1) face-to-face visit between the case manager and the participant every other month in an adult day health care center or the participant’s residence and at least three (3) times a calendar year in the participant’s residence.
10. Issues a participant corrective action plan when appropriate and documents in MWMA.
11. Ensures compliance with all state and federal rules and regulations with regard to HCBW.
12. Maintains excellent assessment skills, clinical knowledge and expertise.
13. Completes the case note in MWMA and Daily Visit Records in a timely manner in conjunction with company policy.
14. Attends branch weekly case conference meeting and submits required paperwork.
15. Is responsible to complete both client and employee supervisory visits according to regulatory guidelines and timeframes.
16. Participates in data collection for Quality Improvement activities.
17. Supports the goal of continuous Quality Improvement by reporting complaints, making pertinent suggestions to improve efficiency, customer service and satisfaction, and/or cost containment.
18. Follows organizational policies and procedures regarding infection control, disposal of hazardous waste, and use of protective devices as evidenced by documentation and absence of related incident updates.
19. Support the principles of Corporate Compliance Program, attends orientation and in-service updates as directed.
20. Completes all other duties as assigned.
