CARE NAVIGATOR - HOME CARE Nursing Hours: 8am-4:30pm Days - Full-time Paygrade/Wages/Salary Info: OH 2013-76 - 40177D
Job Summary Identifies and oversees care management and coordination activities for patients requiring Home Care. # Job Responsibilities Works with all clinical teams as a resource on care management of all patients of the practice including pre-visit planning workflow to ensure care completion prior to visit and after-visit summary review with patients when appropriate. Involves the patients in activities to improve their health (patient engagement) and educates patients about self management tasks they can undertake to gain greater control of their health status. Discusses home care disposition with patient, caregivers, case management team members, and intake team members to identify barriers and opportunities to improve patient experience. Manages assigned panel of Home Care patients. Documents in the patient#s electronic medical record (EMR) and communicates information needed to care team to provide a delay-free and error-free transition from the hospital to home. Provides telephone advice per protocol and handles urgent and emergent calls. Anticipates the needs of patients referred to home care. Necessary planning and pre-visit planning is completed before patient is discharged home. Responsible for working with patient and patient#s care team to coordinate change readiness, needs assessment, and develops an individualized plan of care. Assists patient in setting SMART goals for self-management, teaching them how to do self-management tasks, and report abnormal findings to their physician team. Assesses barriers when patient has not met treatment goals, is not following treatment plan of care, or has not kept important appointments. Increases awareness of home care services and benefits of these services to the patients and providers throughout the region served. Participates in networking opportunities and system meetings that support the goal of improved transitions of patient care. Provides clinical follow-up with patients per protocol when indicated. Qualifications Associate#s degree or higher or Diploma in Nursing required upon hire No experience required RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire AND Driver - Valid driver#s license and 21 years of age required upon hire AND BLS - Certified as a Basic Life Support Healthcare Provider required within 45 days of employment in this position Skills and Attributes Requires critical thinking skills and decisive judgment. Works under minimal supervision. Must be able to work in a stressful environment and take appropriate action.
