Position Summary:
Strive Healthcare is dedicated to providing superior care for patients with kidney disease. The social worker will provide social services to patients and their families in a timely and efficient manner to order to promote maximum social functioning and psychological adjustment to dialysis and rehabilitation of the patient. Services provided will follow the Code of Ethics for Social Worker
Educational Requirement:
• Holds a master’s degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education; or
• Has served at least two years as a social worker, one year of which was in a dialysis unit or transplantation program prior to September 1, 1976, and has established a consultative relationship with a social worker who qualifies under § 494.140 (d)(1).
• Licensure requirements for master-prepared social workers in clinical practice vary from state to state. The social worker must meet the licensure requirements in the state of practice.
Experience:
• Worked at least two years as a social worker, one year of which was in a dialysis facility or transplantation program.
Summary of Duties:
The goal of Social Services is to promote social functioning, psychological adjustment, and overall well-being in each renal patient. This is to be accomplished through:
• Assess the patient’s financial, medical, social, and familial needs.
• Meet the patient’s needs through direct supportive counseling or through referrals to other agencies.
• Recommend changes in treatment based on the patient’s current psychosocial needs.
• Provide case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
• Participate in team review of patient progress and complete monthly care plans on patients.
• Participate in continual quality improvement activities.
• Attend monthly Quality Assurance Performance Improvement (QAPI) meetings.
• Hold initial meetings with all new patients within two weeks or seven treatments from patient’s admission date.
• Complete psychosocial assessments on new patients within 30 days or 13 treatments from patient’s admission.
• Complete psychosocial updates annually or more often as needed.
• Discuss advance directives with patient/family members.
• Perform casework and counseling and hold conferences with patients, families, and support network.
• Intervene with goal-directed counseling. This is a state requirement for this position.
• Help the patient with psychological adjustment and provide support.
• Offer patient counseling and refer him/her to other agencies for resources as necessary.
• Discuss vocational services and complete the vocational rehabilitation form.
• Document each case in medical charts. This is a state requirement for this position.
• Provide pre-transplant counseling for patients and family members.
• Make arrangements with transplant center.
• Participate in staff in-services regarding to patient sensitivity issues and communication.
• Complete required surveys or information required by the ESRD Network.
• Provide information and referrals, including community agency referrals.
• Advocacy on patients’ behalf within the setting and with appropriate local, state and federal agencies.
• Provide patient and family education.
• Complete monthly reports: Clinic’s patient census, Medicaid transportation list, individual provider list, and ESRD-patient activity report.
• Coordinate transient arrangements.
• Arrange transportation services.
• Provide insurance information on new patients and transients and provide updates to the business office.
• Supervise the licensed social worker.
• Fax, copy, file, and assemble admission folders.
• Target patient population: elderly, disabled, illiterate, and socio-economically disadvantaged patients.

• Because of possible contact with infectious diseases, good safety and personal hygiene habits are important. Because of potential exposure to environmental hazards, caution must be exercised to prevent injury. Injuries could be serious.
