Job Title: Home Health Patient Care Coordinator
Immediate Supervisor Title: Operations Manager
Job Supervisory Responsibilities: None
Schedule: 8:30am-5:00pm Monday-Friday
General Summary: A nonexempt position responsible for nursing care under the supervision of providers and the clinical nurse leader.
Essential Job Responsibilities:
• Acts as the liaison between patients and physicians. Talks directly to home health infusion patients on the telephone, gathering information regarding any relevant symptoms in order to identify the severity of their illnesses. Confers with physicians as needed.
• Sending/receiving patient medical records from facilities and home health organizations.
• Complete the Home Health Intake form.
• Create a Medical Record for all infusion patients.
• Obtain lab/x-ray reports, hospital notes, etc.
• Complete forms/requisitions as needed.
• Monitor all laboratory studies for all patients receiving infusions per the protocol.
• Schedule follow up appointments for infusion patients.
• Obtain patient demographics.
• Update charts in a timely fashion per protocol.
• Maintain accurate and confidential records by documenting assessments, interventions, patient/family responses, medications dispensed/prescribed, and test results in electronic health record.
• Reports test results following clinic protocols.
Education: High school diploma. Graduation from an approved practical nursing program preferred. Medical Assistant Certification accepted
Experience: Minimum one year of LPN experience preferred; clinical experience preferred. Minimum of one year MA experience accepted.
Other Requirements: State-licensed practical nurse or Medical Assistant Certification in good standing.
Performance Requirements:
Knowledge:
• Medical terminology, anatomy, physiology, and pathophysiology. Familiarity with health care systems, regulations, policies, and functions. Understanding of documentation standards.
• Equipment, supplies, and materials needed for medical treatment. Understanding of basic laboratory procedures including preparation and screening.
• IDATB policies and procedures related to respective job functions.
Skills:
• Reading and following written and verbal medical orders.
• Initiating appropriate emergency procedures.
• Time management, multitasking and prioritizing.
Abilities:
• Work effectively with diverse patient populations, physicians, other employees and external agencies.
• Read and interpret physicians’ orders/notes.
Equipment Operated: Phone, computer, medical charts, the Electronic Health Records (EHR) system.
Work Environment: Position is in a medical office environment. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.
Mental/Physical Requirements: Involves sitting, lifting and standing associated with a normal clinical environment.

Pay: $15.00 - $19.00 per hour
