Overview
This position is to be represented by a registered nurse who interacts with patients and their care givers primarily through onsite hospital face to face and telephonic encounters. The Transition Case Manager RN monitors patients throughout their hospital stay to ensure that optimal care is delivered in an efficient and cost-effective manner.  The main goal of the Transition Case Manager RN is to ensure patient’s discharge needs are met and continued beyond the hospital stay through a collaboration with Sanitas Medical Centers’ Specialized Care Team (SSCT) that will assure the highest quality primary care is delivered. The Transition Case Manager RN will achieve their essential role by focusing on the physiological, psychosocial and safety needs of each patient and their families.
Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• • Visits hospital admitted patients to establish a nurse-patient relationship and provide and introduction to the Sanitas’ team that will be continuing their care after discharge
• Discusses cases with hospital staff (hospital's assigned case manager) to review and plan services and resources for the patient
• Aims to build rapport with the hospitalized patients and care givers during hospital stay
• Review hospital’s established nursing clinical interventions and care plans to ensure patients’ needs are being met
• Ongoing communication and monitoring of case status with patient and caregiver through different mediums
• Creates and initiates the patient's Clinical Pathway Plan towards efficient discharge and include post discharge continued care goals
• Discusses cases and advises patient needs with the MD Lead in charge of the Sanitas’ Specialized Care Team (SSCT)
Qualifications
KNOWLEDGE, SKILLS AND ABILITIES:
• Must have passion for education and excellent listening skills
• Maintain standards for professional nursing practice, demonstrate the ability to use the nursing process, and formulate nursing care plans
• Must always be able to maintain patient confidentiality
• Must always have excellent oral and written communication skills in English and a second language.
• Must demonstrate ability to manage time efficiently and work independently while also as a team to achieve goals
• Proficient software computer skills to include WORD, EXCEL and other Microsoft Office programs
• Must be detail-oriented to ensure accuracy of documentation.
EDUCATION AND EXPERIENCE CRITERIA:
• Active & clear Registered Nurse (RN) license in the state of Florida required
• Active CPR-BLS required
• Graduate from an accredited school of Nursing, Bachelor of Nursing preferred
• 1+ years Case Manager experience highly preferred
• Fluently Bilingual English and second language required
• Frequent local travel is required
• Experience with electronic medical records (EMR), eClinicalWorks preferred
