Overview
This position is a Registered Nurse (RN) who performs patient health education at their assigned primary care facility through a unique care program that allow patients to achieve self-management. Together with a Care Promoter Medical Assistant (MA), and under the direction of the Lead Care Program Nurse, the Care Educator (RN), plans, coordinates, and holistically monitor patients’ needs to positively impact their chronic health condition through education. The Care Educator (RN) interacts with patients though a variety of mediums that include onsite face to face, telephonic encounters, and group workshops. The Care Educator (RN) might also possess clerical responsibilities of clinical supplies and vaccine inventory management. This position collaborates with a multi-disciplinary team to assure the highest quality care while focused on the physiological, psychosocial and safety needs of each patient and their families.
Responsibilities
• Assist with the identification of patients who can qualify for care program
• Perform direct and group patient education.
• Reviews patient health risk assessments to formulate a nursing care plan.
• Complies with documentation requirements of the Care Management program by re-evaluating the care plan with the patient and family/caregiver(s).
• Data entry in excel report of patient health information and or supervision of such report.
• Preform patient follow up and monitoring according to patient health risk status as stated in program policy.
• Promotes patient self-management of disease and behavior modification interventions.
• Educates patient and family/caregiver(s) about relevant community resources.
• Shares professional knowledge and learning with other team members.
• Takes appropriate action to maintain a safe and aseptic environment.
• Confers with other members of the healthcare team regarding patient care and work in collaboration towards non-compliant population.
• Adheres to department policies and procedures.
• Participates in staff and department meetings on monthly basis.
• Flexible to changes in work schedule and location as needed by organization.
• Communicates to the Care Program Nurse Lead the performance status of their facility’s care program productivity and paraprofessionals working with him/her.
• Supervise vaccine and clinical supply orders and inventory as needed per facility.
• Maintains effective and efficient use of equipment and supplies.
• Attend and assist in company health fairs and events
Qualifications
KNOWLEDGE, SKILLS AND ABILITIES:
• Must have passion for education, portray excellent listening skills, and
• Maintain standards for professional nursing practice and demonstrate the ability to use the nursing process
• Must always be able to maintain patient confidentiality
• Must always have excellent oral and written communication skills in English and a second language.
• Must demonstrate ability to work independently with patients while also as a team to achieve program goals.
• Proficient software computer skills to include WORD, EXCEL and other Microsoft Office programs
• Must be detail-oriented to ensure accuracy of reports and data.
• This position requires require some local travel to assist in covering other facility within the same area.
EDUCATION AND EXPERIENCE CRITERIA:
• Bachelor of Nursing preferred or Graduate from an accredited school of Nursing
• 2 years care management/patient education experience is preferred
• Experience with electronic medical records (EMR), eClinicalWorks is a plus
• Active and clear State of Florida Registered Nurse License
• CPR-BLS Required; ACLS preferred
