Statement of Purpose: The Linkage Navigator will facilitate direct and immediate linkage to on-site medical care as well as other wraparound supportive services, promoting continuity of care and positive health outcomes. Linkage Navigators serve as the point of contact for all individuals seeking to engage in services with Metro or needing assistance connecting with other community resources. Linkage Navigators will facilitate connection to services based on patient choice and need, providing information on program eligibility criteria, documentation requirements, and cost of services (if any) depending on service and program. Linkage Navigators identify and address any barriers to care (i.e. transportation, cost, etc.), ensuring individuals are accessing vital services without delay, advocating and providing assistance to meet the individualized needs of clients and patients.
Primary Tasks/Responsibilities:
• Provide linkage/connection to all programs and services at Metro and within the community including but not limited to medical care, social services, health and wellness programming, etc.
• Maintain updated and available internal and external referrals and resources
• Completing brief needs assessment (via phone and face-to-face) for all walk-ins and call-ins who inquiring of services
• Provide education on available services and any/all requirements to programs and care (i.e. program eligibility criteria, required documentation, etc.)
• Meet with clients/patients who walk in and/or all newly HIV reactive individuals
• Work collaboratively with Medical and Behavioral Health providers, community partners, and other in house staff to meet individual clients/patent needs
• Combat any identified barriers to care (i.e. transportation, insurance status) by connecting the client/patient to supportive services to eliminate or ease these deterrents
• Work strategically to find solutions and resources for clients/patients in difficult situations
• Maintain required records, files, and forms in an organized manner and maintain confidentiality in accordance with HIPAA protocols
Education/Professional:
• Education: Bachelor’s degree in social services
• Minimum of 1 year of work experience within a social service setting or equivalent
Knowledge, Skills and Competencies Required:
• Value for and ability to deliver excellent level of service and working in a team environment
• High degree of professionalism, attentive to detail, and Strong organizational skills
• Self-directed and flexible to meet the needs of the project, organization and target population
• A passion and commitment to support the agency’s mission
• Microsoft suite and data systems comfortability, including Electronic Medical Records
• Ability to recognize and adhere to professional boundaries and standards.
• Ability to handle multiple tasks simultaneously and prioritize responsibilities
• Flexibility to adapt to changing environment.
• Skilled in use of de-escalation techniques.
• Ability to work independently and as part of a team.
• Ability to work effectively and efficiently in a fast paced environment.
• Strong interpersonal skills.
Requirements:
• Must possess and maintain valid Florida driver’s license and proof of insurance
• Must have reliable and accessible auto vehicle.
• Must pass necessary fingerprinting, Level II background checks and employment eligibility verification through the U. S. Department of Homeland Security’s E-Verify system, https://e-verify.uscis.gov/emp .
