Description: The Lead Case Manager of Pathways Hillsborough shall provide supervision of case managers and direct services to support ESG, Up & Out and Rapid Re-Housing Programs in Hillsborough County. The Pathways Hillsborough Lead Case Manager will provide social services and assistance to improve the social functioning of the program clients and to maximize their well-being.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Supervise and coordinate daily staff activities to ensure responsibilities and caseloads are evenly divided.
• Provide staff training on various funding sources requirements and case management needs
• Develop additional training and refresher training as needed
• Interact with program manager to relate client and staff needs, progress of Pathways Hillsborough programs, problems and goal setting and implementation
• Maintain and update the referral resources for staff and client use.
• Oversee record-keeping and effective follow-up of clients by staff
• Represent the Pathways Hillsborough program in appropriate meetings; public forums, workshops, etc.
• Provide all services related to the City of Tampa ESG Program
• Recruit and supervise interns, assist interns with needs, programs, problems and goal setting and implementation
• Interview clients individually, in families, or in groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs.
• Develop and review service plans in consultation with clients, and perform follow-ups assessing the quantity and quality of services provided.
• Refer clients to community resources for services such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provide concrete information, such as where to go and how to apply.
• Counsel clients regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, childcare, or medical care.
• Collect supplementary information needed to assist client.
• Complete and update case records for each family in accordance with HUD Guidelines
• Motivate clients and their families to achieve economic self-sufficiency soon after involvement in program
• Schedule client monthly appointments and additional appointments as needed.
Other Responsibilities:
• Complies with all applicable training requirements.
• Complies with all company safety, personnel and operational policies and procedures.
• Complies with work schedule to ensure effective operations of Agency programs.
• Contributes positively as a member of a productive and cooperative team.
• Participates in Agency Performance Quality Improvement (/PQI) program and Accreditation/Reaccreditation process.
• Performs other duties as necessary to fulfill Catholic Charities Diocese of St Petersburg, Inc Mission.
• Participate in ongoing education through collegiate education, conferences, webinars, national, state and local organizations and or community outreach
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
• Able to speak, write and understand English.
• Must be sensitive to and respect cultural diversity amongst clients, staff and volunteers and able to work with diverse racial, ethnic and economic groups.
• Flexible work schedule including evenings, nights, weekends and holidays.
• Ability to set appropriate limits, work under deadlines and multi-task.
• Ability to organize, prioritize, self motivate, and deliver results.
• Excellent communication and listening skills.
• Possess strong work ethics.
• Successfully pass a FDLE Level II background screening.
• Valid Florida driver’s license, ability to drive van or bus and have reliable transportation.

• A high level of openness and willingness to receive feedback/suggestions from superiors and others, and to learn new skills to improve job performance.

EDUCATION & EXPERIENCE:
• MSW or BSW degree
• 3 years’ experience as a Case Manager.
• Strong leadership and networking skills.
• Commitment to work with diverse refugee populations.
• Thorough knowledge of community services and target populations.
• Bilingual fluency in Spanish and English preferred.
