Qualifications:
• Master's degree or above in Social Work (CSWE accredited) or related field in any health or human services (psychology, sociology, public health, counseling, etc.); Social Workers hired before 2009 who have Bachelor degrees in other fields are grandfathered in; Social Workers hired before 4/21/2019 who have BSWs are grandfathered in
• Minimum of one (1) year experience in health care setting; preferred hospice or hospital
• Minimum of two (2) years' navigation/coordination experience for a group of clients
• Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
• Experience in children, family, and group modalities
• Excellent communication (verbal and written) in English and interpersonal skills
• Ability to work in a team environment and intervene appropriately
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload including supporting telephonically ill patients and families
• Flexible work schedule including the ability to work weekends
• Strong skill level to effectively coordinate patient care for 40 - 70 patients
• Proven successful management of multiple classifications/categories/disciplines
• Strong goal/outcome orientation with exceptional time-management skills
• Excellent customer service focus

Job Responsibilities:
• Manages a population of patients in need of assistance to manage care at home by offering telephonic support and advanced care planning.
• Coordinates services within and external to Chapters Health System including referrals and coordination with community resources.
• Monitors and follows up with patients referred from the Admission Team who are found to be not eligible for initial services assessed for or who are not ready for the services.
• Provides follow up and transitional services on discharge or referred patients providing continuous care to meet patient needs.
• Coordinates care of palliative care clients to ensure the coordination and access to other Chapters Health and external resources.
• Completes detailed documentation of navigation activities.
• Responsible for understanding all of Chapters' Health Service lines to include, but not limited to, what each line can provide, eligibility requirements, treatment guidelines, and needed information to start services.
• Develops strong telephonic relationships; coaches patients and family to understand disease state and how to improve overall health
• Develops understanding of insurance benefits available to patient and navigates insurance resources.
• Utilizes knowledge of services and excellent customer service skills to communicate effectively with patients, families and external resources.
• Performs other duties as assigned.
