DACCO Behavioral Health, Inc. is searching for a Care Coordinator as we transition to a Certified Community Behavioral Health Center! This is a new position in Tampa, FL best-suited for those with experience and passion for the Behavioral Health Field. The ideal candidate will have knowledge of the CCBHC model, understand the full continuum of care, and demonstrate high organizational abilities.

Position Description:
As a Care Coordinator, you will assist patients throughout treatment at DACCO, ensuring the best possible care. You will be responsible for performing initial consultation to identify patient-specific needs and goals, educating patients on Agency programs and options available, and developing a patient care plan in coordination with other Agency providers. You will also coordinate internal and external substance use, mental health, and/or physical health services for each patient in accordance with their individualized care plan and provide updates as the patient progresses through treatment. In addition, you will work with the patient's clinical team to evaluate assigned interventions and treatment to ensure congruity with the care plan and initiate updates and additional interventions when needed. Finally, you will be required to complete standard reports and documentation in accordance with Agency standards.
This is a full-time opportunity, Monday-Friday, with typical hours 8:00am-5:00pm. Some flexibility with the schedule may be required.
Position Requirements:
• Ability to engage and motivate patients
• Superior written and verbal communication skills
• Strong time management and organizational skills
• Knowledge of addiction, preferred
• Proficiency in Microsoft Office Suite
• All personnel are required to undergo a mandatory post-employment offer criminal background check
Required Experience:
• 2-4 years of experience in a Behavioral Health setting
• Direct experience with patient plan coordination, a plus
Required Experience:
• Bachelor's Degree in a Human Services field
• Master's Degree in a Human Services field preferred
• Bilingual-Spanish preferred
