Case Management includes assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, and entitlement to assist severely disturbed adults, youth and their families.
Directions for Living has embarked upon implementing and imbedding Trauma Informed Care principles throughout our agency. Candidates should have knowledge and/or interest in assisting Directions for Living carry out this implementation.
DUTIES
This position includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual, which reflects strengths and self - identified goals. As an integral part of case management services, discharge/transition services are arranged or coordinated by the case manager as an individual is preparing for discharge and transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, or school attendance.) All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs. Timely, accurate and focused documentation is a key performance indicator for this position. Team member may be required to perform other duties as needed. Must demonstrate effective working relationships with other departments and possess a willingness to be flexible. QUALIFICATIONS
• BA in counseling, psychology, social work or other related field required.
• Excellent customer service and people skills required.
• Minimum of 1 year experience working with children experiencing serious mental illness.
• Working Knowledge of clinical/medical terminology a plus.
• Team player & ability to work with others well.
• Experience working with electronic record, preferred.
• Computer literacy required.
• Initiative and willingness to learn a must.
• Knowledge of DSM IV and ICD-9 diagnostic codes.
• Ability to work independently, to coordinate multiple tasks, prioritize work and manage time to accomplish tasks with multiple interruptions.
• Excellent organizational skills, detailed oriented.
• Must have and maintain clear driving record, maintain required personal automobile insurance limits and undergo annual vehicle inspection.
• Must successfully complete background screening process to be a successful candidate.
• Must be able to meet all accountability standards of the position.
• Must have reliable vehicle and may be required to transport clients as needed and assigned.
• Must be able and willing to work in offsite settings including client homes and other community locations.
• May include evening hours.
