• The WSH Transition Coordinator/ PASRR Evaluator is responsible for providingservice coordination to adults who are being discharged from Western StateHospital. The goal of this position is to establish a level of engagement withindividuals returning to RiverValley Behavioral Health region upon discharge.In addition, this position will perform Pre-Admission Screening and ResidentReview (PASRR) responses and evaluations for the RVBH region. May carry assigned Adult TCM caseload underthe Senior Director of TCM or Waiver CM caseload under SD Waiver Case Manager..
Minimum Qualifications:
+ BA/BS degree in psychology, sociology, social work, or other human servicesdegree as approved by DDID and Medicaid to provide Targeted Case Management andDDID Case Management.
AND
+ Minimum of one (1) yearexperience (could be volunteer and/or family member) with individuals withintellectual and/or developmental disabilities and/or ABI

Relocation is not provided and travel is occasionally required
