Overview
The Chronic Care Manager’s primary responsibility is to provide telephone chronic care management for the patients (beneficiaries) of Imperium’s provider practices. The Chronic Care Manager is responsible for triaging, coordination, documentation, communication and tracking of CCM patient calls, cases and records. The goal of the care manager is to facilitate care between provider visits in order to prevent hospital readmission or repeat Emergency Department visits.
The care manager will behave in a professional manner and consistently demonstrate and promote the values of LHC Group. The care manager will work with the practice lead physician to develop this position to best serve the needs of the patient panel and the primary care team.
Additional Details
• Initiates phone calls to assigned beneficiaries monthly to manage/monitor care with the sum of the monthly interactions to accumulate no less than 20 minutes per beneficiary
• Maintains transitions of care between care providers (primary care provider, hospital, Emergency Department, rehabilitation, etc.)
• Coordinates the sharing of patient information outside of the providers office
• Engages patient and family/caregiver in activities to improve the patients’ health by creating a relationship with the patient and family/caregiver as an integral part of the care team
• Provides educational material (written and verbal) for patient, family and caretakers regarding patient's conditions, treatments, and care plans
• Maintain strict confidentiality; follow HIPAA regulations
Qualifications
License Requirements • Licensed nurse (RN, LPN) or certified Medical Assistant
Skill Requirements • Minimum of 1 year medical experience in a physician office. Prefer experience doing telephonic assessments
• Experience with Electronic Health Records
• Experience working with multiple different Electronic Health Records a plus.
• Excellent written, verbal and listening communication skills
• Ability to work independently, with limited supervision
• Proficient computer skills-data entry, retrieval and report generation
• Ability to work with a diverse patient/family population
• Ability to document in multiple platforms including a CCM platform and multiple EMR’s
• Ability to work with a VoIP telephony system
• Ability to handle urgent and emergent calls and provide telephone advice according to protocol
• Working knowledge of Medicare billing and regulations
• Working knowledge of ACO/APM, MIPS, and quality reporting
• Must have a room that can be dedicated to performing job functions that can insure patient/business privacy and a distraction free environment
• May be required to participate in an on-call rotation dependent upon licensure.
• Familiarity with multiple EHRs, NextGen experience preferred.
